First Time Clients: Please download and bring the completed intake packet and privacy notice found below to your first appointment for each person attending the session.
For your convenience, we have two sets of forms to download and bring to your first visit. This will allow you time to fill out the forms at your leisure and give us optimum time for us to get to know each other on your introductory appointment.
Just click on the links shown in blue below and to the right and download both sets of forms so you can print them and fill them out to bring the completed documents to your first visit. Please fill out one set of forms for each person attending the session.
** if you have a child under the age of 18 please fill out the permission for treatment of a minor as well.
